Uses:

  • Treatment of hypertension

  • Reduction of stroke risk in hypertensive patients with left ventricular hypertrophy

  • Patients inadequately controlled with monotherapy

Dosage:

Hypertension

  • Usual starting dose: 1 tablet once daily
  • Maximum:
    • 2 tablets 50/12.5 daily OR
    • 1 tablet 100/25 daily
  • Full effect after ~3 weeks

Severe hypertension

  • Start 50/12.5 once daily
  • May increase after 2–4 weeks if needed

Renal impairment

  • Use if creatinine clearance >30 mL/min
  • Not recommended in severe impairment

Hepatic impairment

  • Not recommended for dose titration

Can be taken with or without food.

Side Effects:

Common:

  • Dizziness

  • Edema

  • Palpitations

  • Abdominal pain

  • Back pain

  • Cough

  • Sinusitis

  • Rash

  • Upper respiratory infections

Warnings & Precautions:

  • Monitor electrolytes regularly

  • Risk of fluid or electrolyte imbalance

  • May precipitate gout

  • May affect blood glucose control

  • Monitor renal function when combined with NSAIDs

Pregnancy & Lactation:

  • Pregnancy category D (avoid use)

  • Present in breast milk; discontinue drug or breastfeeding

Drug Interactions:

  • Potassium-sparing diuretics or supplements → hyperkalemia risk

  • Lithium → increased lithium levels

  • NSAIDs → reduced antihypertensive effect, renal risk

  • Other antihypertensives → additive effect

  • Alcohol, barbiturates → orthostatic hypotension

Contraindications:

  • Hypersensitivity to components

  • Anuria

  • Sulfonamide hypersensitivity

  • Concomitant use with aliskiren in diabetic patients
