Azithromycin can interact with several common drugs, notably those that prolong the QT interval, such as amiodarone, sotalol, quinidine, dofetilide, haloperidol, citalopram, and fluconazole, increasing the risk of life-threatening arrhythmias. It may also enhance the effects of warfarin, raising the risk of bleeding, and elevate digoxin levels by altering gut flora, potentially causing toxicity. Additionally, azithromycin can increase the levels of immunosuppressants like cyclosporine and tacrolimus, and may interact with statins (e.g., simvastatin, atorvastatin), raising the risk of muscle toxicity. Close monitoring or dose adjustments may be needed when these drugs are used concurrently.
