Kolzine Tablets

Composition

Each tablet contains 500mcg of colchicines. Colchicine is an alkaloid obtained from colchicum autumnate and other (Colchicum) spp. Colchicine is useful in prevention of acute attacks of gout and in the relief of the symptoms of acute gout.

Uses:

Colchicine is used for the relief of gout and the prophylaxis of acute attacks. Colchicine produces dramatic response in acute gout probably by reducing the inflammatory reaction to urate crystals. This effect may be due to several reactions including decreased leucocycte mobility. Colchicine has no analgesic effect and no effect on blood concentrations of uric acid, and no effect on the excretion of uric acid. Colchicine possesses antimitotic activity.

Dosage:

The recommended dosage is 1mg initially, then 0.5mg every 2 to three hours until pain relief is obtained or gastrointestinal adverse effects occur. The total dose should not exceed 10mg. At least 3 days should elapse before another dose is given.

When used for the short-term prophylaxis of gout doses by mouth range from 0.5mg to 0.6mg one to three times daily.

In Mediterranean fever (recurrent polyserositis) doses of 0.5mg to 2mg daily may be given prophylactically; a dose of 0.6mg every hour for 4 hours then every 2 hours for a further 2 hours has been suggested for amyloidosis.

In primary billiary cirrhosis 1 to 1.2mg daily in divided doses has been recommended, while in Behcet’s syndrome up to 1.8mg daily in divided doses has been suggested. Patients with pericarditis may respond to doses of 0.5mg to 1mg daily.

Side Effects:

The most serious adverse effects of colchicines; diarrhea, nausea, vomiting, and abdominal pain, are often the first signs of toxicity and are usually an indication that colchicines therapy should be stopped. Others include bone marrow depression with agranulocytosis, thrombocytopenia and aplastic anemia following prolonged treatment. Peripheral neuritis, myopathy, rashes and alopecia may occur.

Warnings & Precautions:

Elderly and debilitated patients especially those with renal, GI or heart disease or hematological disease should use colchicine with caution. Dosage should be reduced if weakness, anorexia, nausea, vomiting or diarrhea appears.

Safety and efficacy in children has not been established.

Pregnancy & Lactation:

Colchicine can cause foetal harm when administered to a pregnant woman. Use only when clearly needed and when the potential benefits outweigh any potential hazards to the foetus.

It is not known whether colchicine is secreted into breast milk.

Drug Interactions:

Colchicine induces reversible malabsorption of b12. Colchicine is inhibited by acidifying agents and potentiated by alkalinizing agents. Colchicne may increase sensitivity to CNS depressants. Response to sympathetic agents may be enhanced.

Contraindications:

Colchicine is contraindicated where there is hypersensitivity, serious GI, renal, hepatic or cardiac disorders and blood dyscrsias. Impaired hepatic function may increase toxicity. Colchicine inhibits spermatogenesis.
