Mode of action

Aspirin has an analgesic, anti-inflammatory, and antipyretic actions. Aspirin acts by inhibiting the enzyme cyclo-oxygenase, which results in the inhibition of biosynthesis of prostaglandins. Prostaglandins are associated with development of pain that accompanies injury of inflammation. Enhanced production of prostaflandins is a common feature in fever. Aspirin also inhibits platelets aggregation.

Paracetamol is an effective antipyretic and analgesic, it produces andipyresis by acting on the hypothalamic heat regulating center and analgesia by elevating the pain threshold.

Caffeine caffeine in lower doses stimulates the CNS and this imparts a synergistic effect to the aspirin-paracetamol combination

Over dosage

Over dosage may cause dizziness, tinnitus sweating, nausea, mental confusion, hyperventilation, ketosis or coma.

Treatment for over dosage

For mild intoxication emptying the stomach emesis or aspiration or gastric lavage. Prompt administration of 50g activated charcoal and ½ litre iced mannitol reduces absorption. In severe intoxication the risk of severe liver damage can be significantly reduced by the administration of methionine 2.5g by mouth every four hours unit 10g has been given or IV 20% N-acetyl cysteine should be administered.
